How to Write Nursing Care Plan in NANDA-1 Format
A common reason care plans come back marked down isn't the clinical content. It's the way the nursing diagnosis is phrased, because a vague diagnosis makes everything that follows harder to justify. Take a post-surgical patient with reduced mobility. A diagnosis written as "patient has difficulty moving" is too vague to build a care plan around. It doesn't say why , and it doesn't give the rest of the plan anything specific to respond to. A NANDA-I formatted version of the same diagnosis might read: "impaired physical mobility related to surgical incision pain, as evidenced by reluctance to reposition independently and reported pain score of 6 out of 10 on movement." Notice what this version does. It names the problem (impaired physical mobility), gives a cause (surgical incision pain), and gives evidence (reluctance to reposition, pain score of 6/10).
